Individualized Service PlanIndividualized Family Service Plan (IFSP):
When a child is determined to be eligible for services, and the family has decided they would like to receive Birth to Three services, an IFSP is developed. Family members and providers work together as a team to develop a plan that explores family routines and how we can provide services that support their child’s development and improvement in those routines. During home visits our staff will focus on the coaching style of interaction for parents and caregivers. This will help the family be comfortable and knowledgeable about their child’s development and will enhance the child’s learning within family routines and activities. |
Parent/Caregiver Coaching Style of Intervention Coaching as a style of interacting is provided by our trained staff and includes:
Observation Joint Planning Action Feedback Reflection TransitionOur staff works closely with family members, schools and other professionals to develop appropriate transition plans for those children entering the school system when the child turns three and exits Birth to Three.
We will evaluate children upon request from school systems as needed. Our goal is to best meet the needs of each child and their family to ensure a comfortable and successful transition to school or other community settings |
